Lead Paint Forms are available at the Vermont Departmentof Health website.
For those who use Trueforms, you can access the new lead paint forms and handouts by updating your Trueforms. If you use automatic updates, simply login into Trueforms and click update. If you update manually, login to Trueforms, click on Tools and click update.
